Our story

Housing is often the missing piece

Too many capable adults leave prison, a hospital, or a shelter with nowhere steady to go. A missing address can stall a job, a benefits application, or a parole plan before it starts.

We opened Collective Care Homes to close that gap: a clean, furnished home with clear expectations, where residents can steady themselves and build toward permanent housing. We work alongside case managers, payees, parole and probation officers, and families so that everyone supporting a resident stays on the same page.

We provide housing, not care

Collective Care Homes is an independent shared-living residence. We are not a personal care home, assisted living facility, or nursing provider, and we don't provide medical or personal care.

That means residents need to manage their own hygiene, meals, medication, mobility, and housekeeping. If someone needs hands-on help, we'll say so up front and do our best to point you somewhere better suited. A resident may bring in a licensed home health agency at their own arrangement, and we'll cooperate fully with that provider.

Residents get structure and practical help toward work, income, documents, and permanent housing. We sit down with you, go through the paperwork together, and help you take the next step.

One rate per person covers your furnished room, all utilities, and Wi-Fi. No surprise bills, so residents and payees can plan with confidence.

Your rate, payment schedule, and any one-time intake fee are confirmed in writing at intake.

What a resident needs

Independence

An adult who is ambulatory and manages their own hygiene, meals, medication, and mobility.

Intake & agreement

A completed intake and screening, plus the signed house rules and program agreement.

ID & payment source

A photo ID or acceptable identification, and a verified way to pay. Need help getting an ID? We can help with that.

For referral partners

A dependable housing resource for your clients

For case managers, social workers, hospital discharge planners, parole and probation officers, VA staff, and ministries. When housing is the last barrier to a good transition, call us.

Refer when your client

  • Is ready for discharge or release and no longer needs your facility's level of care
  • Is ambulatory and manages their own hygiene, meals, medication, and mobility
  • Wants a clean, structured, drug- and alcohol-free home
  • Has income, a payee, an agency, or a sponsor to cover the monthly rate
Not the right placement

Please look elsewhere if your client

  • Needs help bathing, dressing, transferring, or toileting
  • Needs medication administered or supervised
  • Is not ambulatory, or is a fall risk requiring supervision
  • Has an unstable psychiatric or medical condition
  • Has no payment source in place

We respond the same day. If someone isn't a fit, we'll tell you quickly and try to point you to someone who is.
